Sir,
I, Chandrakala Ramchandra Yadav, r/o Room No. 149, Mulund Colony, Shivaji Tekdi, Hanuman Pada, Hindustan Chowk, Mulund, Mumbai 400 082  holding permit no. 96/E/1010 and my taxi vehicle no. MH03-F-4880.
I had taken loan from Bobby Enterprises and you had taken the signature on the blank agreement paper, receipts & RTO forms from me.  It is pertinent to note that copy of the original registration book and permit are also in your custody.  My taxi vehicle is 1983 model.  It is pertinent to note that I had paid entire loan amount to you through my driver and you had issued original receipts on the name of my driver instead of my name.
It is further pertinent to note that you had issued various receipts on the name of Bobby Enterprises as well as Singh Motors, the reason best known to you.
It is further pertinent to note that this is nothing but clear that it is criminal breach of trust & cheating with me to issue such receipts.
Under the above mentioned facts and circumstances, I request you to please clear your position that how and under what circumstances & intention you had issued the provisional receipts of 2 firms when I had taken the loan on the mortgage of my old taxi vehicle from M/s. Bobby Enterprises.  Please also clear that how and under what intention you had issued the provisional receipts on the name of my taxi driver instead of loan borrower i.e. Chandrakala Ramchandra Yadav.                                                                                                    …2
